All animals are given a 24 hour oral medication with alter, however, we strongly urge all animals being spayed & neutered purchase additional pain medication to help w/ pain & inflammation.  This will help reduce chances of complications.  Thank you.

    ANIMAL CARE & CONSENT  
     
    I, the below signed, understand that uncontrollable circumstances can occur with any kind of surgery where an animal has to be anaesthetized (put under, or to sleep).   I also understand that because the Hotaling Spay/Neuter Clinic offers this spay/neuter program at a much-reduced price, blood-work and other such preliminary tests cannot be run, and therefore, pre-existing conditions in my animal(s) may not be known. Because of this special low-cost situation, I agree to place no blame on the Hotaling Spay/Neuter Clinic or any participating Veterinarian. I will not hold the Hotaling Spay/Neuter Clinic responsible for problems which may occur after I have brought my animal home, even if such problems require my animal to see a veterinarian and incur a fee while there.

    The BG-WC Humane Society and its agents reserve the right to refuse to perform procedures on an animal for any reason. All animals will be tattooed to show proof of alter. No exceptions. 

    The Hotaling Spay/Neuter Clinic agrees to take care of your animal to the best of our ability while it is in our care and to return your animal in the same shape it was when dropped off for surgery (minus its reproductive organs of course!). Thank you for choosing to alter your pet, as it is a life saving surgery, not only for your pet, but also for the millions of unwanted animals that are born each year. 
     
    Please sign below to show that you have read and understood this contract.

    POSSIBLE ADDITIONAL CHARGES: 
    If my animal is IN HEAT, PREGNANT, or having a false pregnancy there is an additional charge of 20.00.
    If my animal is
    crypt-orchid (meaning that one or both of the testicles have not dropped) there is an additional charge of 35.00.
    If my animal has a
    pyometra (infected uterus) there is an additional charge of 35.00.

    All of these conditions require a longer amount of time and anesthesia, therefore costing more. These prices DO NOT include any additional pain medications or antibiotics recommended by our veterinarian. Additional charges must be paid when the pet is picked up from surgery. 

    While it is not always the case, small breed dogs can be prone to having retained baby teeth (teeth that have not fallen out when the adult teeth came through). Our staff is trained in this and can remove these teeth to help prevent any future dental diseases. I give permission for the staff to remove these teeth and incur an extra charge while my pet is there. The charge is normally approximately 20.00. If it will be more than 20.00, our staff will call you ahead and let you know. 

    CANCELLATIONS / REFUNDS POLICIES:  
    If for any reason this appointment must be cancelled, below are the stipulations: 
    ***NO REFUNDS will be given after 45 days of this application 
    ***These funds may NOT be transferred to any other use (adoptions, retail sales, etc.)
    ***A 10.00 service fee will be deducted from the refund to cover bookkeeper, postage, paperwork fees.

Once you submit your application, please allow 24-72 hours response and one of our staff will contact you for payment via credit card over the phone and to schedule your appointment.  Applications received on Friday will be reviewed on Monday due to the clinic not being open on the weekends.

*Veterinary services are not available every day. Please call ahead to confirm that a Vet is available.*
Our Vet normally does examinations on animals between the hours of 10 a.m. and 2:00 p.m. on Tuesdays and Thursdays 
Other services may be available between 10 a.m. and 4 p.m.
